Health cover with a small NIRP

A Brit friend is about to retire here, from Jamaica, with a small NIRP from work in the UK decades ago. He has worked in Jamaica for many years and has health cover from his university scheme there (as does his wife, who is Jamaican and has MS).
He is going to be pretty skint, even without paying anything avoidable for health cover in France. I normally try to research these things for myself, but their circs are so far different from mine that I would be grateful for pointers from anyone who has been anywhere close.
